Transition planning is the ongoing process of preparing students with disabilities for the adult life they choose after high school. A truly successful transition process is the result of comprehensive team planning that is driven by the dreams, desires, and abilities of students. A transition plan provides the basic structure for preparing an individual to live, work, and play in the community, as fully and independently as possible. This training will assist families in understanding the transition planning process specifically looking at what components of the IEP are essential as the students plans for their future. Transition Planning Participants will gain insights into:
o Assessing their child's strengths, interests, and needs.
o Establishing measurable post-secondary goals and annual goals.
o Identifying appropriate transition services and coordinated activities.
o Understanding the various Diploma pathways & credentials offered to students.
All trainings are free to attend, but individuals must be enrolled in our FSS Training Program to register.
