Please join us for Gifford’s annual Advanced Care Planning Fair where you can complete or update an advance directive and have questions answered around advance directives and long-term care planning.Gifford staff will be on hand to answer questions and assist with paperwork.
A number of community partners and service providers will also be present for you to interact with. Community partners will include:
