About this Event
The Annual SoCal Rehab Golf Classic is San Diego's only golf tournament that pairs able body golfers with players who have limitations due to an illness or physical impairment. The purpose of this event is to highlight ability not disability while enjoying the comradery and the challenge of being out on the course.

Mission
Alvarado Hospital's San Diego Rehab Institute's SoCal Rehab Golf Program promotes golf for Seniors and individuals who have a physical disability.
Fundraising for the Community
Monies raised will support the Alvarado Damas de Servicio. Tax ID#237210063. The auxiliary is a non-profit, philanthropic and service organization, which has raised money to support health education and patient care projects since its founding in 1972. The Auxiliary supports PALS, Programs for Active Living and Support. These are post treatment programs for the benefit of individuals with physical disabilities and senior citizens. They include, SoCal Rehab Golf Club, Stroke Peer Visitor Program, Comebackers Neuro Club, the Alvarado Balance Club (ABC), the Aphasia Group, t he Senior Fitness Class, the Book Club, the Support Group, the Back Nine Seniors, and SDRI's Art for All Program.
Golf Classic Format
The 9-hole tournament is a four-player scramble format. A team will be comprised of at least one individual with a physical limitation. Golfers with physical impairments will have the option to play from designated forward tees.
Players are invited to register individually, in teams, or as an agency team vying for the Alvarado/SDRI Jug. Qualifications for Alvarado/SDRI Jug: Healthcare agencies and corporations are invited to register a complete foursome with a minimum of one individual with a physical challenge. The winning team in this special category will be awarded the coveted Alvarado/SDRI Jug.
Tournament Registration*
Includes greens fee, cart, goodie bag, golf apparel, one opportunity drawing ticket, one awards dinner ticket, one beverage ticket, and the eligibility to win hole-challenge prizes (Putting Challenge, Longest Drive, Closest to the Pin, Closest to the Line, and Hole In One).

Consent and Release of Liability for Participation in SoCal Rehab Golf Classic
As a participate in the SoCal Rehab Golf Classic organized by Alvarado Hospital, I understand the program will be not be directly supervised by Alvarado Hospital. I agree to abide by the rules, regulations, and all restrictions of the Hospital. I understand that golf, the use of various types of golf equipment, transportation to and from the golf center, as well as simply being present in the recreation area, exposes the participant to risk of physical injury. I have been encouraged and have the opportunity to discuss these risks in view of my physical limitation, medical condition and/or medications with my physician.
I hereby release Alvarado Hospital, its agents, servants, and employees of any liability for injury, illness or damage to myself, or loss or damage to property as a result of my participation in the SoCal Rehab Golf Program.
I hereby absolve the Riverwalk Golf Club their employees/staff from any responsibility, should an accident, illness or injury occur while on the property of their golf facilities.

Photographs and video will be taken at this event. By entering the event premises, you consent to photography, audio recording, video recording and its/their release, publication, exhibition, or reproduction to be used for social media, news, marketing materials, advertising, inclusion on websites, or any other purpose by Prime Healthcare and its affiliates and representatives. You release Prime Healthcare, its partners and each and all persons involved from any liability connected with the taking, recording, digitizing, or publication of interviews, photographs, computer images, video and/or sound recordings. If you do not wish to be photographed please inform an event staff member.

If you are not feeling well prior to or on the day of the event please stay home and get well. Please notify us as soon as possible.
