About this Event
Our annual Awards Gala and Golf Tournament enables the Foundation to administer community programs geared towards the Social Determinants of Health.
The Community Transformers Foundation was established to advance health and social equity. The Foundation seeks to address the social determinants of health, promote wellness and equality in communities that are historically underserved and underrepresented.
Community Transformers Foundation is a private independent 501 (c) (3) organization serving communities in the Commonwealth of Virginia.
The annual Golf Tournament takes place on Friday, May 31 at 7:30 AM at the Crossings Golf Club.
