Paperwork Need for "Signing Day" to register your child:
1. Proof of birth (birth certificate, passport, etc.)
2. Vaccine certificate (on the Florida DH680)
3. Physical dated after 8.12.24, including parent side
4. Two current proofs of address
5.Valid email address
